Tesamorelin is a synthetic analogue of growth-hormone-releasing hormone (GHRH), modified with a trans-3-hexenoic acid group that resists degradation by the DPP-4 enzyme responsible for native GHRH's short half-life. Acting directly on the pituitary's GHRH receptor, it stimulates the body's own pulsatile growth-hormone release rather than introducing exogenous GH — a mechanism distinct from direct-acting GH secretagogues like Ipamorelin, which is why the two are often studied together as complementary points on the same axis.
